REGISTERED CARE AIDE - JOB POSTING
The Role:
The Care Aide is an essential member of the Health Link Outreach team. You will be responsible for providing in-home care to patients, while helping them maintain their independence, dignity and holistic well-being.
Key Responsibilities:
- Work closely with the nursing and pharmacy teams to support chronically ill patients
- Administer pre-dispensed medications on scheduled assignments
- Basic vital signs monitoring using automated equipment and assigned documentation
- Participate in ongoing training
- Build rapport with individual patients to develop effective care partnerships
- Monitor and report on patient’s day-to-day progress, symptoms and behaviour
- Maintaining a clean and safe environment for clients and staff

LICENSED PRACTICAL NURSE - JOB POSTING
The Role:
LPNs support clients and their families to achieve independence while maintaining optimal health. You will be responsible for visiting patients in their own homes to provide personal care in a one-on-one setting.
Key Responsibilities:
- Monitor patient’s condition and report on patient progress
- Provide witnessed medication administration
- Administer SC/IM injections
- Educate patients on how to self-monitor their condition
- Track progress of patient’s current morbidities
