REGISTERED CARE AIDE - JOB POSTING

The Role:

The Care Aide is an essential member of the Health Link Outreach team. You will be responsible for providing in-home care to patients, while helping them maintain their independence, dignity and holistic well-being.

Key Responsibilities:

  • Work closely with the nursing and pharmacy teams to support chronically ill patients


  • Administer pre-dispensed medications on scheduled assignments


  • Basic vital signs monitoring using automated equipment and assigned documentation


  • Participate in ongoing training


  • Build rapport with individual patients to develop effective care partnerships


  • Monitor and report on patient’s day-to-day progress, symptoms and behaviour


  • Maintaining a clean and safe environment for clients and staff

The Role:

LPNs support clients and their families to achieve independence while maintaining optimal health. You will be responsible for visiting patients in their own homes to provide personal care in a one-on-one setting.
